Fill out WSDCA Universal Application

Photo IDs for all adult Household Members

Birth Certificates or Medicaid Cards for all children under the age of 18

Social Security Cards for all Household Members

Proof of Income for the previous 30 Days for all working Household Members who are over 18+

Proof of Physical Address

Authorization to Release Information (may be requested by Coach)
* If you are seeking assistance for rent/mortgage or utilities, you will need to apply on the state website: South Dakota Housing
